In today’s time, the role of wisdom teeth is not significant, and it is often heard that they are not even necessary, but this is not entirely accurate. If a wisdom tooth erupts and is positioned correctly in the jaw, if we can clean it well, it will be a strong and durable tooth that can be functionally used, so it is worth preserving and caring for it. However, it often happens that wisdom teeth push the teeth in front of them, or perhaps do not erupt at all, but remain trapped in the bone, that is, they grow in a completely wrong direction. Because they often cause major problems, wisdom tooth extraction is often performed surgically, primarily to avoid problems and complications.

When is wisdom tooth extraction necessary?

Wisdom tooth extraction is usually recommended if the tooth grows in the wrong direction or is not expected to erupt in the mouth based on X-ray images, that is, if it can cause a problem or inflammation. In these cases, if the wisdom tooth remains in the bone, it will push the teeth, which will cause not only an aesthetic but also a functional problem. Maintaining oral hygiene is difficult in the case of crowded teeth, because the space between the teeth becomes so small that even the use of dental floss is almost impossible. As a result, cavities, gingivitis, or periodontitis can easily develop.
Due to their location, they are often inaccessible for cleaning, and if cavities develop, tooth restoration can be difficult, and if adequate treatment cannot be provided, extraction is performed. There are also cases when wisdom teeth do not fully erupt but only partially, where food often becomes trapped between the teeth and soft tissues, causing inflammation, which is also one of the indications for wisdom tooth extraction.

Illustration of why wisdom tooth extraction is a necessary procedure when there is no room for teeth in the jaw.

How does wisdom tooth extraction work?

Wisdom tooth extraction is completely painless. Before extraction, you will have a consultation with the dentist so your doctor can familiarize themselves with your health condition, the medications you take, any allergies. As a first step, after the consultation and analysis of the X-ray image, the dentist will give you anesthesia. When the anesthetic begins to take effect, the tooth extraction proceeds. When the tooth has erupted, it is possible to extract it like any other tooth with forceps.

In case the tooth has not erupted, the oral surgeon will remove the wisdom tooth with surgical instruments.
In our clinic, after tooth extraction, we have the option of placing a PRF preparation. The essence of the procedure is to reduce pain and swelling. This significantly reduces healing time compared to traditional wisdom tooth extraction.

How long does wisdom tooth extraction take?

It is difficult to give an exact answer to this question because the duration of the procedure depends on many factors. Taking the most common case: if we extract two wisdom teeth simultaneously from both jaws on the same side (for example, upper right molar and lower right molar), our patients can expect a procedure of 30 to 90 minutes.

A few good tips on what to do after wisdom tooth extraction

On the day of the procedure, after the effects of local anesthesia wear off, eat only soft, mushy, and chilled food.

Definitely avoid consuming hot food and beverages.

Use cold compresses on your face to reduce pain and swelling.

Physical activity increases arterial blood pressure, which can cause increased bleeding, so we recommend that you do not exercise for a few days. Do not rinse the wound, as this can jeopardize clot formation and thereby cause further bleeding.

If you notice bleeding, bite on sterile gauze for half an hour.

Up to 5 days after the procedure

Be sure to take antibiotics, if prescribed by your dentist, and a probiotic.

Tooth brushing continues normally at least twice a day with a soft toothbrush.

Pain and facial swelling are usually most pronounced after two or three days, especially in the morning. This is completely normal; after four days, the swelling begins to subside.

If swelling persists or increases, contact your doctor.

Make sure you follow all instructions from your dentist/oral surgeon.
